H1N1 FLU CLINICS

H1N1 vaccinations for individuals ages 6 months and older are available on a walk-in basis at the DeKalb County Health Department, 2550 N. Annie Glidden, any week day (M - F), from 8:30 to 11:30 a.m. or 1 to 4 p.m.  No Appointment is necessary.

H1N1 vaccinations are available during evening hours by appointment only by calling 815-748-2467

 

INSTRUCTIONS FOR CLINIC

Clinic will have injectable H1N1 flu vaccine.  Anyone 6 months of age and older can get the H1N1 vaccine.  Vaccines are free of charge.

Minor Children:  Children under the age of 18 must be accompanied by a parent or legal guardian.

Clothing: Wear short sleeves for easy access to upper arm for injectable vaccine.

Wait time:  Expect to be at the Health Department for approximately one-half hour.

Consent Forms:  Consent forms for this clinic for individuals 3 years and older are on the Health Department H1N1 flu website. This form can be printed, completed, and brought to the clinic. Forms for 6 months to 3 years will be available at the clinic.

Vaccination Fact Sheet: Please review the Vaccine Fact SheetsYou will not be given the vaccine if you are:

(1) Allergic to eggs
(2) Had a fever of 100°or greater in the past 24 hours 
(3) Have had a life threatening reaction after a dose of seasonal flu vaccine 

* In addition to the Health Department, vaccinations will be available at a large number of DeKalb County health care providers and pharmacies.  Northern Illinois University also has vaccine for their students.
