Coordinating care with a team of healthcare professionals, our registered nurses help patients recuperate and manage a variety of health conditions surrounded by family support.
Experienced and responsive nurses and therapists, in coordination with the patient’s physician, help patients and their families learn how to provide care.
The Home Care Program is a state licensed and Medicare certified program with over 45 years experience providing home health services in DeKalb County. We are the premiere experts to nurse you back to health and maximum independence.
Home Care provides:
- Skilled Nursing
Nurses provide nursing care and treatments and develop a plan of care with the patient’s physician, including the patient and family in the plan.
- Home Health Aides
For patients receiving skilled nursing, Home Health Aides work under the supervision of the registered nurse to assist with daily routines such as bathing, grooming, dressing and medication reminders.
- Physical Therapy
Physical therapists work with patients to improve gait and mobility, relieve joint and muscle pain and to help restore muscle function through exercises, instruction and other therapeutic treatments.
- Occupational Therapy
Occupational therapists help patients return to the routine of daily living, providing instruction in self care, the use of adaptive equipment and living with disabilities.
- Speech Therapy
Speech therapists help with speech and swallowing problems caused by stroke or other circumstances. In addition, they help with cognitive and memory problems.
- Medical Social Worker
Medical social workers help patients and their families identify community services and provide counseling and case management services.

On call nurses are available after hours and on weekends for home care patients.

We accept Medicare, Medicaid and most private insurance. Please check with staff for further information regarding insurance and payment.
Home care services are available to homebound patients of all ages in DeKalb and surrounding Counties.

Q) Am I eligible for Home Care services?
A) If you have recently experienced the following, you may be eligible.
- Hospital or nursing home discharge
- Frequent hospitalizations in the last year
- Multiple medications, changes in medications and new medications
- Decline in physical function
- A new illness or diagnosis
- A history of falls or safety concerns
- An increase in breathing problems
- Difficulty getting to the doctor
- Loss of a caregiver, or you live alone with medical problems
Medicare requires that you are homebound and you need skilled care. Private insurance requirements vary, but most insurers follow the Medicare guidelines.
Q) What is “homebound?”
A) Homebound means you are unable to leave home without considerable and taxing effort.
Q) What is “skilled care?”
A) Skilled care consists of services that must be performed by a registered nurse, such as:
- Patient and family education about the illness and care
- Injections
- Intravenous (IV) therapy
- Wound care
- Heart and blood pressure monitoring
- Medication instruction and monitoring
- Coordination of care with the patient’s physician and other healthcare professionals
In addition, skilled home health nursing care is the provision of intermittent skilled services. Intermittent means the nurse visits two to three times per week for 45 minutes to an hour, or more frequently if needed.
Q) How can Home Care help me?
A) Home Care can help restore and maintain your maximal level of function and health so you can remain in your home independently.
Q) Do you provide homemakers, companions or other custodial and non-medical care?
A) No, but your nurse or medical social worker can refer you to other available resources.
Q) How can I get Home Care?
A) Talk to your physician or to the discharge planning staff, if you are in a hospital or nursing home. Once we have an order or request from your physician, we can evaluate your eligibility for Home Care services.
Q) Will there be a cost for Home Care services?
A) If your insurance is Medicare or Medicaid, there is no charge to you. If your insurance is a private plan or Medicare Advantage, you may have a deductible and or co-payment. Please check with staff for further information regarding insurance and payment.
In addition, some private insurance plans limit the number of covered visits, and the staff can provide more information about that as well.
